Workers' Comp Forms

Important - Do not file work related injuries on your group medical or prescription plans. File only with the Workers' Compensation Insurance Carrier.

If you are injured while at work:

Report your injury to your supervisor immediately
Complete injury forms
Workers Compensation Form - Packet includes authorization form, prescription card and in network doctor locator tool
Visit In Network Physician as necessary
Notify supervisor and Workers' Comp Coordinator, Jessica Allen ext. 2111, of work status

If you have been injured and do not wish to pursue your incident as a work-related injury please complete this form and email it to jallen@brenhamk-12.net. Election of Remedy
