Get your Physical

We encourage athletes to complete their physical with their primary care physician. 

Athletes can make an appointment to be seen at the BS&W HWY 290 Clinic by calling 979.337.5400.

Athletes can also pay cash at The Brenham Clinic.  Make an appointment by calling 979.337.5800.

Physicals and Action Plans

All completed forms can be scanned and emailed to trainer@brenhamk-12.net or hand delivered to the athletic trainers.

Students with Medical Conditions: 

Athletes and marching band participants who are diagnosed with asthma, diabetes, seizures, and severe allergic reactions that require an Epi-pen will be required to complete additional paperwork found below:

These forms MUST be printed out and completed by a physician. Additionally, athletes and marching band participants who have been prescribed an inhaler, Epi-pen, diabetic supplies or seizure medication will be required to have these items immediately available at all practices, scrimmages, tournaments and games.  If the athlete or marching band participant does not have these items, they are not allowed to participate.  

Most common reasons why a physical is not accepted.  

  • Form is missing a parent or student signature.
  • Form is not completely filled out or is filled out in pencil.
  • Additional form is not provided for asthma, diabetes, seizures, or severe allergic reactions that require an Epi-pen (more information listed above).
  • Vision portion of the exam was completed without prescribed glasses or contacts.

Additional Information

Insurance:  It is recommended that all student athletes have personal health/accident insurance coverage. The school’s insurance will only help with partial coverage of injuries resulting from athletic participation.

 

Office Hours:  Athletic trainers will be in a lot of different places with meetings and practices but we are really going to try and have at least one AT in the office at Kruse Field House during the day.
